There are a number of forms which you will have to fill out and return in
order to have all of your credentials in order. We are enclosing a few of them
with this letter. Please return those which need to be filled in as soon ás
possible.
1. Physical examination form 1193 (return)
2. Immunization form - 2 (return one, keep one)
3. form 615A - Application for employment (return)
4. Insurance statement - 2 (return one, keep one)
5. Designation of insurance beneficiary (return)
6. Insurance conversion regulations.
7. Insurance statement re conditions of employment as
emergency Federal employee.
The assignment which we are offering you pending completion of satis-
factory credentials is for staff nursing. As you will have noticed on the
general information sheet which has been sent to you, the salary for staff
nursing is $110 a month plus room, two meals a day and laundering of uniforms.
Of course, you realize that in accepting an appointment it would be for
the duration and that the Red Cross will assume the responsibility of the
traveling expenses from your home to Hawaii.
