Dear Miss Madley:
Thank you for offering me an appointment to the
nursing staff of the American Red Cross-Harvard Hospital
Unit. I am very pleased to accept it.
I am enclosing the following reports; chest x-ray,
dental and eye. I mailed a copy of the chest x-ray to
Dr. Caughey. He recuested this when he did my physical
examination on January 6th. I had received instructions
for this examination from Miss Phillips.
Also, I consulted Miss Phillips about the require-
ment for a physical examination by my private physician.
This was done last July and a report was sent to the Red
Cross. She told me that she thought that this would be
sufficient. If not, I shall be glad to have it repeated.
My required immunization will be complete next Friday.
I do not have a uniform picture. Therefore, I am
enclosing one taken in street clothes recently. If you wish,
I can have another taken in my Henry Street uniform.
