your commi ttee enrollment record card from our Worcester, Massachusetts

Local Committee to the one in Sacramento, California and will send you an
official advice of this transfer as soon as it is completed. We are sending
ne
a copy of your letter to our Public Health Nursing and Home Hygiene Service
so they may have the information given regarding your work at Simmons College,
et cetera.
We note that you have not brought your records as an American
Red Cross nurse up to date by filling in one of our address questionnaire forms
since 1929 and, therefore, we are taking this opportunity to send you a ques-
tionnaire which we will greatly appreciate having you completely fill in and
return to us for our files. As you k ow we ask each and every American Red Cross
nuise to fill in one of these forms annually, just to keep her records up to
date and make it possible for us to always number her among our Red Cross nurses
in good standing. We will look forward to receiving your completed question-
naire by return mail.
