My dear Miss Petry:
On June 17 we sent you an address questionnaire
to complete and return to National Headquarters in order
to bring your records as a Red Cross nurse up to date for
the current year. We have not yet received the form in
our office and undoubtedly this has just been an oversight
on your part. We are enclosing another form for your con-
venience which we hope you will complete and return to us
promptly.
