Resignation or Release (The following information must be given) Reasons for resignation or release: Should worker be reemployed for same or any other type of Red Cross Service: Discuss worker's ability and personality : Personnel report prepared by Title INSTRUCTIONS COVERING PREPARATION (1) Covers only travel and maintenance from home to place employed or vice versa. If worker is to be reimbursed regularly for travel and maintenance this should be indicated in space provided. Copies shall be made as follows : At National Headquarters: 1 copy for salaries less than $3,000 2 copies for salaries of $3,000 or more. At Area Offices: (a) Less than $3,000, 2 copies shall be made and forwarded to National Headquarters. (b) $3,000 or over, 3 copies shall be made and forwarded to National Headquarters In case of personnel transfers, the responsibility for initiating the pay roll changes shall rest upon the office making the transfer. Salary rate shall always be shown except in the case of a release or resignation. The name of the Appropriation shall be shown as well as Appropriation Symbol. When Temporary Personnel are recruited and assigned by the Manager of an Area to a disaster field operation three copies shall be prepared. See also Personnel Chapter of Disaster Relief Handbook, ARC 234, covering use of this form for Disaster Relief Personnel.
