My dear Miss Leprestre:
It was indeed a pleasure to learn of your interest and
availability for an assignment to care for civilians in Havaii,
The attached application blank, Form 1219, and employ-
ment application, Form 615-A, should be completed and returned to
us immediately, together with a photograph of yourself. I am
also enclosing two copies of Insurance Notice, one of which should
be signed and returned to us for your permanent record, and the
other to be retained by you for your information.
All murson accepted by the Unit must here immunization
for typaoid, totanus and smallpox, and we urge nurses who are inte-
rested in the assignment to bagin with these inmunisations et once,
You understand, of course, that we cannot give you 6. definite
appointment until we have had an opportunity to review your creden-
tials.
There steps include the only action necessary for you to
take at present; but I would suggest that you secure your birth cer-
tificate which you may need in case you are accepted for the Unit.
I assume you have discussed your interest in this assignment with
your employer and have arranged for your immediate release, if called.
You will hear from me promptly after we have had an oppor-
tunity to review your application.
