I have been informed that all valuables and funds in my posses-
sion should be deposited with the hospital authorities.
Any funds or valuables not turned in are retained at my own
risk. I consent to any interest, accruing on deposits made by me,
being transfered to the Hospital Fund. I hereby authorize the
custodian of the Patient's Fund at Letterman General Hospital to
deposit at my own risk in a United States Depository such funds
as he may accept for safe-keeping.
