completed by Mrs. Elizabeth Martin, badge number 33,385 who
gives as her temporary address 132 E. 45th Street, New York.
On the questionnaire, Mrs. Martin indicates that she is planning

to change: her address and employment in the near future.
Will you please send the Number 2 card for this nurse
to National Headquarters in order that it may be properly trans-
ferred at such time as Nrs. Martin has made definite plans.
Please attach a 2802 form to the card when you return it, indi-
cating the purpose for which you are sending it to National.
