THE AMERICAN RED CROSS
NATIONAL HEADQUARTERS
WASHINGTON, D.C.
July 30,1941
Mrs. R. Louise Mellarrus
Main Stroet
Bass River, Wassachusetts
My dear Mrs. Moliamus:
Your application for enrollment in the Red Cross Nursing
Service has been received and we are glad to find that you meet the
requirements for Home Defense enrollment. Therefore, we are pleased to
herewith send you your appointment card and badge, No. H D 3305
which are your credentials as a Red Cross nurse. Will you please acknowl-
edge receipt of these credentials by return mail on the enclosed postal
card, giving your number for verification.
Please read the attached regulations concerning the wearing
of the badge and be careful always to observe them.
We know that you will accept your enrollment in the Red Cross
Nursing Service as an opportunity to maintain high standards of nursing
care wherever nursing is needed under the American Red Cross.
It is important that you keep us informed of any change you
may make in your name or address, in order that we may be aware of your
whereabouts in event of an opportunity for service under the Red Cross.
Each year an address questionnaire will be sent to you either
by the Local Committee with which you are enrolled or National Headquar-
ters. This form should be filled in and promptly returned to the address
given on the questionnaire. Your failure to respond for two consecutive
years places you in the inactive classification in the Red Cross Nursing
Service.
Sincerely yours,
Appointment Card and Badge
Acknowledged 8-2-41
