Sir or Madam The person concerning whom this inquiry is made has applied for foreign service in The American Red Cross. As an aid to it, in the performance of its duty to obtain accurate information regarding the applicant's patriotism and loyalty to the United States, you are requested to answer all of the following questions (or to state after any question which you do not answer your reason for not doing so). Your replies to these ques- tions will be regarded by The American Red Cross as confidential and will not be disclosed either to the applicant or to the persons other than government officials, who, in the performance of a public duty, may be called upon to examine them. What has been the character and extent of your acquaintance with the applicant (business, professional, social) Have known her socially and professionality as a nurse. 7. Have you been asked to endorse the applicant's loyalty, and if so, by whom and under what circumstances? no. 8. What is the general reputation of the applicant? Good to best roots my knowledge. (a) for character and integrity Good Mr so far as know (b) for loyalty to the United States rexcellent (c) for attitude and loyalty to one and all of the Ailies of the United States all right 9. What public service, if any, in connection with the war (i. e., Liberty Loan, Red Cross, Y. M. C. A., War Savings Stamps, etc., etc.,) has been rendered by the applicant? Don't know.
