The person concerning whom this inquiry is made has applied for foreign service in The American Red
Cross. As an aid to it, in the performance of its duty to obtain accurate information regarding the applicant's
patriotism and loyalty to the United States, you are requested to answer all of the following questions (or to
state after any question which you do not answer your reason for not doing so). Your replies to these ques-
tions will be regarded by The American Red Cross as confidential and will not be disclosed either to the applicant
or to the persons other than government officials, who, in the performance of a public duty, may be called upon
to examine them.
