We have recently been in correspondence with
Miss Ada Holden Stafford, No. 29467, enrolled with your Committee,
and upon our request she has completed a questionnaire. It is
dated April 26th and gives her probable address for the next
year as - Central Street, Millinocket, Maine, telephone number - 75.
She states that her health is good. We are sending this informa-
tion for notation on her No. 2 card.
