My dear Dr. Self:
We are referring to you for physical examination, Miss
Janet Elizabeth Stahl, Butterfield Memorial Hospital, Cold Spring,
N. Y., a Red Cross nurse who is interested in being considered for
service in Hawaii. We are now writing her to get in touch with you
for an appointment.
We shall appreciate your cooperation, and will see that
you are compensated at the rate of five dollars ($5.00) for the
exanination.
The physical report, bill and any recommendation for
further examinations should be sent to this office, through which
they will be referred to our Personnel Service.
In appreciation of your cooperation, I remain
