My dear Miss Storey:
As I have received your letter of May 18 I am sending the following
without waiting to have the rpport from your physical examination. You
should, therefore, go ahead with these forms and if there should be any
reason for physical désqualification, I would wire as soon as the physical
examination report is received.
1. Insurance statement (2 copies, one to be retained for your
information, and the other to be returned when signed.)

3. Form 615A (application for employment)
4. Supplementary information
5. Instructions to obtain uniforms (outdoor, indoor and hat)
6. Information on uniforms, clothing, etc.
I have just sent you a telegram giving instructions about reporting
in New York for your uniforms. Will you please send to this office the
following:
1. The address to which your uniforms should be delivered.
2. Your hat size.
3. Your indoor uniform size.
