My dear Miss McCoy:
We have been requested by our Public Health Nursing
and Home Hygiene Service to send you a set of application papers for
enrollment in the Red Cross Nursing Service.
Kindly read the enclosed pamphlet, then fill in the
application form in your own handwriting being careful to answer every
question fully and correctly.
Call the attention of the doctor to the instructions
printed on the physical examination blank, particularly those relating
to vision and hearing and request him to answer the questions according
to the instructions.
The accompanying card should be filled in by yourself.
When the papers are completed please send them to -

Red Cross lursing Service
General Hospital
Cincinnati, Ohio
The necessary credentials will be obtained by the Com-
mittee and the papers will be signed and forwarded to the National Office
for final Consideration and action.
