Herewith please find form conteining facts for declaration of
intention to become en American citizen which was forwarded to this
office for attention.
The date you arrived in the United Stetes ha.s been found from the
Inni retion records. TAKE THIS LETTER and the enclosed paper to the
Office of the Clerk of any court heving neturalization jurisdiction in
the district in which you reside and he will file your declaration of
intention. The Clerk's fee will be $1.00.
No witnesses are required.
