SUMMARY OF PERTINENT CLINICAL HISTORY. PHYSICAL FINDINGS. AND PROVISIONAL DIAGNOSIS Heart size RADIOGRAPHIC REPORT (Use reverse side for additional space) Upper gastro-intestinal series reveals no evidence of an organic lesion involving the esophagus, stomach or duodenum. Spot films of the duodenal bulb were made with and without pressure and no evidence of ulcer or deformity was present. The proximal portion of the je junum was filled and the lumen and valvulae conniventes are of normal size and appearance. Studies of the chest reveal a small healed primary complex in the right upper lobe. The heart and aorta are within normal limits in size and there is no enlargement of any of the individual cardiac chambers. A small fat pad is present in the left costophrenic angle, FILM NO.
