535 Pearl Street
New York City
Dear Mr. Barker:
Enclosed are four bills submitted by
Mrs. Emma Hussman, who was in to see me today. I have
okeyed them. They total$6.39. She states that the
amount of the vouchers which she sent in May was $23.61.
In addition I would request that you include $5.00 for
her transportation charges for today.
Her general condition is good, but she is
having considerable disability at present in the right
knee and back. She has recently lost one of her upper
teeth which became so loose that she was able to extract
it herself. Another tooth is loose and is to be extracted
tonight. On the right lower jaw there is a sluggish
abscess which has to be opened from time to time. There is
a discharge from both ears.
Very truly yours,
Lloyd F. Craver
Attending Physician
