HEADLINE: Kosar, Cowboys agree to terms; Quarterback is candidate to start against
Cardinals

IRVING - The Cowboys agreed to contract terms with Bernie Kosar late Tuesday, and the
former Cleveland Browns quarterback will be on the Valley Ranch practice field with Dallas
on Wednesday afternoon, sources said.
Kosar, released by the Browns on Monday, cleared waivers Tuesday at 3 p.m. Early in the
day, it appeared that the Miami Dolphins had his services locked up, but the Cowboys
entered the contest in the afternoon and beat out Miami, Kansas City and Philadelphia for
his services.
With Troy Aikman doubtful because of a pulled hamstring, it's possible Kosar will be in
the starting lineup against Phoenix rather than backup Jason Garrett, who has never made an
NFL start.
Hugh Millen apparently will be given his release by the Cowboys on Wednesday.
Contacted at home by Cowboys public relations director Rich Dalrymple, coach Jimmy
Johnson declined to comment on the situation Tuesday night. Johnson spoke highly of Kosar
during his Tuesday morning news conference but was so pessimistic of signing Kosar based
on salary concerns, he said there was a better chance of the Cowboys signing "anybody in
this room" than Kosar.
However, Kosar's recently signed contract with Cleveland contained guarantees from the
club for 1993, lowering his contract demands from his new club. It's not certain what Dallas
will pay Kosar but it's far above the league minimum, a source said. Kosar's contract with
Dallas will be for one year, allowing him to become a free agent after the season.
Cowboys owner Jerry Jones was out of town Tuesday night and unavailable for comment.
Kosar also could not be reached for comment as he was in conference with his father, who
serves as his agent.
Kosar, 29, played his final season at the University of Miami for Johnson, who was
replacing Howard Schnellenberger that year.
Johnson always has been a supporter of Kosar's and had positive things to say about him
at his news conference.
"I think Bernie Kosar is a talented player," Johnson said. "I think he can win football
games. There's no doubt in my mind Bernie Kosar can help this team. There are a lot of
teams in this league he can help. I think he can probably help some more than others."
Kosar helped the Browns reach the playoffs in each of his first five seasons. That included
three trips to the AFC Championship Game, where Cleveland lost all three times to the
Denver Broncos.
Kosar signed a seven-year, $ 26 million contract extension last month but has had a rocky
relationship with head coach Bill Belichick. He fell completely out of favor this season
despite leading the team to a victory over the San Francisco 49ers in Week 2.
Kosar was benched in favor of Vinny Testaverde, but Testaverde was injured against
Pittsburgh on Oct. 24. Kosar returned to the starting lineup for Sunday's game with Denver
but was released Monday.
Browns owner Art Modell said Tuesday he was unaware of the Cowboys' interest in Kosar.
"If he goes to Dallas, fine. I wish him well. He's a dear friend of mine," he said.
Modell declined to say what his contractual obligation was to Kosar for this season but
said it's less than the reported $ 5.35 million. "I think maybe there is a small offset where if
another team signs him there would be a deduction from my obligation to him," Modell said.
Modell thought Philadelphia and Miami had shown the most interest in Kosar. The rumors
of Kosar going to the Dolphins were very strong. Former Cleveland tight end Scott
Galbraith, who worked out for the Cowboys on Tuesday, said he talked to Kosar for 15
minutes by telephone. "He's going to the Dolphins," Galbraith said.
Kosar is noted for his intelligence and is likely to pick up the Cowboys' offensive scheme
quick enough to play Sunday should Aikman continue to be unavailable because of a pulled
hamstring.
Aikman is listed as doubtful.
Kosar's final pass as a Brown on Sunday was a 38-yard touchdown pass to Michael
Jackson.

Accompanying this memo is basic information about University Hospitals
of Cleveland, and our pediatric hospital, Rainbow Babies and Childrens
Hospital. Mrs. Clinton will be spending her time at "Rainbow," as it
is generally known.
As I mentioned, Farah M. Walters (President and Chief Executive
Officer of University Hospitals) and Mrs. Clinton know each other from
the work Mrs. Walters did on the health care reform task force, and
Mrs. Walters sat with Mrs. Clinton's party in the Capitol when the
President presented the health care proposal to the Congress on
September 22.
Also: Secretary of Health Donna Shalala visited here Thursday,
February 18, the day after the President's State of the Union
address. (See attached clip, with photo of Secretary Shalala and Mrs.
Walters from The New York Times.)
Note: The hottest topic in Cleveland right now -- and one that Mrs.
Clinton might want to mention -- was the release of highly popular
Cleveland Browns quarterback Bernie Kosar, who yesterday became a
Dallas Cowboy. There may be some some lighthearted way in which this
could be worked into a conversation with a child patient.
I have spoken with Karen Robinson, your advance person. We will be
identifying children (patients) and their families. Total of
approximately 10 people will meet with Mrs. Clinton. The topic is
"children and health."
Attached is a list of people who will officially greet Mrs. Clinton,
and other physicians/staff who we have asked to be present.

The program offers a unique benefit design -- triple option, point-of-service -- allowing
enrollees to select how they access services, either using their Primary Care Physician (PCP)
or deciding to pay more for self-referrals to specialists, or opting to obtain services outside the
contracted network. Benefit payments are made on the basis of cost differentials associated with
different access alternatives enrollees select each time they require health services.
A Primary Care Physician - who serves a medical manager of patient care -- has been
selected by 98% of QualChoice enrollees, 50% of whom selected a PCP for the first time. This
means initial contact with the health care system is at the primary care level - -- the least costly
encounter setting. Yet satisfaction levels (93%) were as high as those reported in national
surveys for enrollees in non-managed care environments. In fact, in the QualChoice model,
satisfaction level measurements actually increased as use of the plan increased.
By giving physicians clinical decision-making freedom linked with financial accountability
for results, physicians are incented to achieve quality outcomes in the most cost-effective
manner. This is demonstrated in hospital utilization rates superior to most HMOs and PPOs.

enrollees were 316 in 1991, 301 in 1992, and 298 in 1993 which favorably compares to the
average Ohio non-Medicare HMO rate of 377 per 1,000.
The result of the QualChoice model has been to reduce costs through innovative,
aggressive medical management. First year clients enjoyed a 14.2% cost reduction compared
to predecessor plans. In subsequent years, the program, continuing to apply medical

management and utilization improvement, significantly moderates the impact of medical cost
trends. Second year and third year results reflected year over year cost increases of 7.8% and
6.7%, respectively.
Through establishment of a partnership between hospitals, physicians, and employer plan
sponsors, a collaborative, non-adversarial environment has been created. With sophisticated
analytical tools, QualChoice has been able to identify best practices, superior clinical decision-
making, and progressive medical management protocols.
The reordering of physician incentives permits the restoration of clinical freedoms, but
now with a shared commitment to both cost-effectiveness and quality outcome.
