Dear, Madam:-
We have on file at this office the application of
Mary Agnes Bartley who is at present in England. Miss Bartley states
she graduated from your training school on May 13, 1913. We have
a record at this office that wa sent to your Hospital for
credentials on October 29, 1917 and again on December 5, 1917.
We have had no reply to these requests and I num wondering whether
they have not been miscarriede Sinoe Miss Bartley's anrollment
cannot be accomplished until we have a training school credential,
I em requesting that you fill out the enclosed blank at your
earliest convenience and forwarded it to this office. Any in-
formation you wish to give will be considered strictly confidential.
