My dear Mrs. Brophy:
Our Fort Wayne, Indiana Local Committee on Red
Cross Nursing Service, which has been carrying your records as
a Red Cross nurse, recently forwarded the same to this office

with the information that you have failed to respond to the
annual address questionnaires sent you since April 1927. In
this connection, we must call your attention to the marked
paragraph on page 7 of the enclosed pamphlet, A.R.C. 703,
explaining our ruling in regard to the annullment of the en-
rollment of those nurses who failed to keep in touch with the
Red Cross and from whom we have been unable to get a response
for two years or longer.
We hope you are still interested in your enroll-
ment as a Red Cross nurse and if so, feel sure you will be only
too glad to cooperate with us by filling in the enclosed ques-
tionnaire form and returning it to us promptly, so that your
records may be up to date and you may be numbered among our Red
Cross nurses in good standing.
