questionnaire completed by -Miss Mary Isabell Campbell,

number you have for Miss Campbell is 28083.

Gampbell, #36586, and Mies Mary J. Campbell, {28083, are
not identica and it is. the latter nurse who is enrolled
with your Committee. Miss Mary Isabell Campbell, #35586,
is enrolled with our Camden, New Jersey Local Committee.
