INSTRUCTIONS FOR PREPARATION OF THIS FORM

Authority for travel or advances shall be approved in accordance with existing regula-
u
tions before forwarding to the Department of Accounting.

The proposed itinerary, giving dates of departure, arrival at various points en route, and
the purpose of each visit to be made shall be shown.
The upper portion of the form shall not be filled out when the purpose of the advance
is for other than travel or when the advance is for travel purposes and a travel authority
is not required in accordance with the Travel Regulations section of Travel and Leave Regu-
lations (ARC 508).
All employees traveling for the first time shall be furnished with a copy of Travel and
Leave Regulations (ARC 508) and instructed to render accountings in accordance with
these regulations.
