Please refer to your letter of December 13, 1941, to Mrs. George

H. Evans, 37 Mansfield Street, Everett, Massachusetts, with which you
forwarded to her for execution "Claimant's Statement" and "Claimant's
Receipt, Release and Agreement," preliminary to the payment to her by
the Metropolitan Life Insurance Company of insurance due Phyllis L.
Evans under our Group Life Insurance Policy No. 8869 G.
So far as my records show, these Forms have not been executed and
returned by Mrs. Evans and unless perhaps you have received them or have
received some communication from Mrs. Evans indicating that she does not
intend to execute them, I would suggest that a follow-up letter be sent
to her.
