I, the undersigned, an applicant for a passport to
enable me to go to SIBERIA under the auspices of the American Red Cross,
solemnly ewear that I have no near male relatives serving in the armed
forces of the United States or serving overseas in a civilian capacity
attached to the American Expeditionary Forces or to a relief or hospital
organization, except Georgest Forsten U.S. Engineers
Gehtendung, Curruary Frederick C. Forster -ambulance -
French any of Geoupation Speeper Bararia (Both. are birther)

further solemnly ewear that I am going to SIBERIA
solely as a Red Óross worker; that I will make no effort to visit my
brothers under any conditions, and that it is not my intention to merry
an officer or soldier in the American Expeditionery Forces after my
arrival in SIBERIA.
