S + June 2, 1942 AIRMAIL Miss Stephanie Fannie Foto 2712 Clairmont Avenue Birmingham, Alabama F My dear Miss Foto: We have received your wire indicating your availability for service with the American Red Cross Unit in Hawaii, and are enclosing forms for your physical examination and immunization vaccines. These should be filled out and returned to us at once. 1. Physical examination form 1193 2. Immunization form The report of your physical examination will, of course, have a great Jeal to do with your acceptance for the assignment and we expect that you will have a thorough examination, as a foreign assignment would be unwise unless your physical condition is excellent. Please take the responsibility of impressing your physician with this fact/ Select your own physician and inform him that the Red Cross is prepared to pay for your examination, not to exceed $5.00. His bill, if there is one, should be submitted to this office in care of the Nursing Service./Note the required immunization. Tetanus toxoid is also required but can be arranged when we notify you that you have been definitely accepted. Yellow fever vaccine will be administered at the port of embarkation. Your teeth should be in good condition so that it will not be likely that you will need dental work. The assignment which we are offering you gending completion of satis- factory credentials is forranesthetist with the understanding that you will serve as a general staff nurse/if necessary./ or emergency We do not as yet have definite word of the date of the Unit's leaving although it is possible that we may be asked to have the nurses ready on rather short notice. Even though it would be necessary for you to bemalkking tentative arrangements for release, you should not give up your position until you have not only received a definite appointment from us, but have been notified of the date on which you should be ready for assignment.
