Madam:
Your nomination as nurse in the U. S. Public Health
Service has been recommended to the Department with com-
pensation at the rate of $720 per annum, quarters, sub-
sistence and laundry, to be effective from the date of
entrance upon duty.
You are requested to preceed July 1, 1920, to U.S.
Marine Hospital /11, Portland Ave. a 22nd St., Louisville,
Ky " and report for duty to the Medical Officer in Charge.
While executing these orders, you should aecure re-
ceipts for all money paid out, to present with your travel
account for reimbursement.
By direction of the Surgeon General.
