I C & 101 TS, M 7911 August 16, 1919. Miss Mary A. Hickey, and 148 Hancock St., D Springfield, Mass. 9 Madam: 5 (b Your nomination as nurse in the U. S. Public Health S Service has been recommended to the Department with compen- sation at the rate of $720 per annum, to be effective from the date of your departure from Springfield, Mass., with quarters, subsistence and lanndry. There are enclosed transportation requests covering your railroad am Pullman fare, receipt of which you directed to adchowledge by number. You are directed to proceed immediately to U. S. Marine Hospital, Baltimore, Md., and report for duty to the Medical Officer in Charge. You will also report to the Bureau the date of your departure from Springfield, Mass., and your arrival at Baltimore, lid. A later communication will be received (from Civil service commission), a copy of which should be attached to your account for reimbursement of expenses, confirming this nomination and authorizing your travel. By direction of the Surgeon General. Respectfully, Assistant Surgeon.
