dear Miss Johnson:
Miss Havey has advised us of your letter of
August 11th to her, regarding your change in address and your
desire to have your record card as a Red Cross nurse transferred
to our Maryland State and Local Committee on Red Cross Nursing
Service with headquarters in Baltimore.
Upon consulting your records as a Red Cross
nurse in our files, we refret to find that you have not brought
them up to date by filling in one of our annual address question-
naire forms since 1932 and, therefore, we are taking this oppor-
tunity to request you to fill in a questionnaire for us at this time.
Enclosed is a form which we will appreciate having you complete
and return at your very earliest opportunity and upon its receipt
we will see to it that your transfer to our Maryland State and
Local Committee is effected.
