Unit transferred to O.C.D. in the Territory of Hawaii

INSTRUCTIONS COVERING PREPARATION
(1) Covers only travel and maintenance from home to place employed or vice versa. If worker is to be reimbursed regularly for travel
and maintenance this should be indicated in space provided.
Copies shall be made as follows:
At National Headquarters: 1 copy for salaries less than $5,000 2 copies for salaries of $5,000 or more.
At
Area Offices: (a) Less than $5,000, 2 copies shall be made and forwarded to National Headquarters.
(b) $5,000 or over, 3 copies shall be made and forwarded to National Headquarters.
