Angle,\n1200 Huron Building,\nKensas City, Kansas.\nMy dear Dr. Angle:\nWe have just been edvised by Miss Carmen Hayes, Secretary to\nDr. Algie, that he is now in service with the Navy.\nWe recently referred a nurse, Miss Betty Jean Koopman, to him\nfor a physical examination to deteraine her physical fitness for ser-\nvice.\nOur Assistant Red Cross Medical Advisor, Dr. Amos Christie,\nhas advised that we contact you for this examination. lie shall appre-\nciate your cooper tion, and will see that you are compensated at the\nrate of five dollars ($5.00) for the examination. We are now writing\nMiss Koopaen to get in touch with you for an appointment.\nA physical examination report form is herewith enclosed.\nIf on examining this nurse, it is your recommendation that special ex-\naminations (such as X-ray, electrocardiogram, basal metabolism, etc.)\nare indicated, will you please communicate with us for authorization.\nThe physical report, bill and any recommendation for further\nexaminations should be sent to this office, through which they will be\nreferred to our Personnel Service.\nIn appreciation of your cooperation, I remain\nVery truly yours,\nVirginia M.
