Subject: Discharge from Red Cross Military Service.
1. The undersigned Alyce Nella Lockrey served in the Red
Cross Military Service during the World War, but has never received
an official discharge certificate.
2. Some of the facts connected with my service are as
follows:
Name served under: Alyce Nella Wormwood
Red Cross Military Service-Oct 2, 1917 to Sept 16,1918.
Trans. to Army Reserve Corps Sept. 16, 1918
Appointed to continue with Army Reserve Corps, Evacuation
a
Hospital no. 110 at Coiney, France, Major John Moorehead, C.O.
Lieut. Benjamin McGrath, acting adjutant, gave the oath.
Sept. 16, 1918.

Hospital moved to Villers- Dancourt, France Sept.27, 1918.
Unit recalled and demobilized Nov. 24, 1918.
Hospitalized at Hospital no. 101 at Neuilly, France
for the treatment of bronchitis. Upon discharge from hospital, I re-
ported to Army Nurses Hdq. in charge of Carrie N. Hall, where I was
informed that the office of the Surgeon General had not received my
papers, and I was refused travel orders.
The Red Cross then paid my salary from Sept. 16, 1918 to
the date of my release, Dec. 28, 1918, and sent me home to Boston, Mass.
3. Some of the officers connected with my service: Dr. John
Moorehead, 115 East 64th St., New York City. Dr. Terr W. Homer ,HARMER
Marlbourough St., Boston, Mass. Dr. Benjamin R. McGrath, Connor Bldg.
Grand Island, Nebraska. Arthur E. McLendon, 1330 Assembly St., Colum-
bia, South Carolina.
4. It is requested that I be given a discharge certificate,
or certificate in lieu thereof.
