My dear Miss McNutD:
We have received your application for assignment to Hawaii and
are able to offer you a definite appointment for general staff duty.
I am therefore, enclosing further forms which will give us in-
formation which is necessary in the 08.30 of nurses assigned outside the
mainlande
1. Application for employment.
2. Designation of beneficiary - Please add age of beneficiary
for identification purposes.
3. Disposition of salary.
4. Insurence notice - 2 copies - One copy to be signed and
returned with above form.
5. Personnel investigation card.
6. Red Cross caps - 3
Red Cross nurse's am insignia - 3 under separate cover.
Red Cross nurse's cape - cotton - 1)

When these are received please sign the card and return.
Do not hold up return of other forms for this if package

should be slow in reaching you.
To be sent later:
Advance of money for travel expenses.
We are just today sending in nemes of nurses definitely appointed in
order to obtain final word regarding arrengement for transportation.
It is difficult to say how soon you may be called, but we certainly
expect to be able to give you notice one week in advence. As there is
little possibility of reservation by air, it will be necessary to proceed
by train to the coast. Further information will be sent the minute it is
available.
