C. 20006 Dear Katy: Thanks for your help with the chapter listing and was so glad to hear from you. We all send you our best wishes and congratulations on your assignment in Washington. The above nurse has contacted our office reference to perhaps becoming active again as a Preparation For Parenthood Instructor. She states she took the instructor course at our chapter in 1958 or 59. I find no records in the files that go that far back. Our question is how can she become reinstated as an instructor and can a copy of her Form 2824 be sent to us if it is on file in Washington? She did receive her badge through our chapter, but again I find no record of this. She states she never had her records transferred to any other area.
