We all send you our best wishes and congratulations on your\nassignment in Washington.\nThe above nurse has contacted our office reference to perhaps becoming\nactive again as a Preparation For Parenthood Instructor. She states she\ntook the instructor course at our chapter in 1958 or 59. I find no records\nin the files that go that far back.\nOur question is how can she become reinstated as an instructor and can a\ncopy of her Form 2824 be sent to us if it is on file in Washington? She\ndid receive her badge through our chapter, but again I find no record of\nthis.
