We have had great difficulty in getting the correct
addresses of enrolled Red Cross Nurses in our territory. We
know that you are very busy, but the National Committee re-
quires us to submit, annually, a complete list of all the
nurses enrolled with our committee, with addresses up-to-date.
Will you kindly answer the following questions and re-
turn this form to me in the accompanying self-addressed envelope
at the earliest possible date?
