My dear Miss Amick.
In response to your recent letter received
December " 1912, I am sendingin to you the
pink form with necessary information.
It will be convenient for me to
leave my present position May , 1943 or June 1,1943
I am in public health work. now, it will be
necessary for me to get all white uniforms need
suntable clothing.
I shall be ready to leave at the date
best suited. and will have physical examination
as directed
