Dear Madam: As you undoubtedly know, the National Committee on Red Cross Nursing Service requires us to submit, annually, a list of all the Red Cross Nurses enrcl- led with our Committee, with their latest addresses. We know that you are very busy, but we are extremely anxious to make a very complete report to National Headquarters and hope for your cooperation. In this connection your attention is called to the fact that the War Department will NOT hold a NATIONAL DEFENSE TEST DAY ROLL CALL THIS YEAR, but in lieu thereof will ACCEPT THE RESPONSE TO OUR ANNUAL ;ADDRESS QUESTIONNAIRE, Form 1045. You are, therefore, urgently requested to send in this questionnaire at once, The fact that you are not available does not excuse you from sending in this form de simply state that you are not available, and send in the question- naire with the same promptness as if you were ready for service. We want to have your correct address, and the filling out of this questionnaire is a requirement of each enrolled Red Cross Nurse. Will you kindly answer the questions on the reverse of this sheet, returning it to me in the enclosed, stamped addressed envelope.
