The aplication for foreign service of
the following nurse enrolled in your Division has been re-
ceived by this office. May I request that you instruct ner
to secure a popy of her birth certificate and have one (1)
dozen photographs taken, the head size to be 1 by 3/4/inches
with no hat on, and that you send her instructions contained
in Paragraph #9 of the Division Routine.
If this nurse cainnt secure a of
her birth cersificate she should secure an affidavit from
some meraber of her family older than herself, stating place
and date of birth. This she should retain to present to the
Clerk of the Court when making application for passport.
Applioant should see that the Clerk of
the Court promptly forwards the passport application to the
Bureau of Foroim Personnel, American Red Cross. Washington,
D.C. and that there is attached to the application when so
forwarded one dollar in bash, one signed photograph, one un-
signed photograph, and on birth certificate or affidavit of
birth.
