Miss Carolyn Lucille 0*Connor indicates that
she has been employed at your institution at some time

within the last two or three years, although she did
not anxious indicate to serve definite, with dates the Army of Nurse employment. Corps and She we is are
very
writing to ask you for a special report in regard to her
service in your institution.
We are especially ansious to know about her
general nursing ability and her personal suitability as
it relates to her emotional stability, conduct and
character.
Any information you can give us in regard
to her récord with you will be greatly appreciated and you
may be sure will be held strictly confidential.
