The attached application blank, Form 1219, should bo comploted
and returned to us immodiataly, and the physical blank, Form
1193, le to be used by the exanining physician. Arragements have bean
made by Kethleon Leaby, Chairmen, Local Committee on Red Cross Nurs-
ing Service, University of Washington, Soattle, to have a phywician maice
the examinations in your territory, and 1 suggest that you get in toüch
with her immediately. It le important that you plen to have your exenina-
tion at once, mailing the completed blank via airmail to me without delay.
I es also attaching tiro copies of Insurance Notica, one of which should be
signed and returned to me for your permenent record, and the other to be
retained by you for your information.
All nursos accepted by the Unit mut have immunation for
typhold, tetanus and mollpox, and we urge nurses who are interested in
the assignment to bagin with these immunizations at onos. You understand,
of course, thet wo ennot give you a definite appointamt until we have
had un opportunity to review your cradentials and havo recoived & report
of your physical
