Shields, Gays Mills, 5-4-32 appt. letter, card and Wisconsin. badge sent to Miss Van Kooy, Chr., Madison, Wis. Local Com. 3 My dear Miss Shields: Under separate cover we are mailing your appointment card and badge, No. 54,490. Will you please acknowledge their receipt on the postal card enclosed with the badge, and for verification kindly give number? We find that you meet the requirements for enrollment as a Red Cross nurse. This means that you are a graduate of an accredited school of nursing connected with a hospital caring for a minimum daily average of fifty patients; in the event of your having graduated from a small school you have supplemented your training by affiliation or by post graduate courses; you are registered; a member of the American Nurses' Association; and a citizen of the United States. We expect you to continue to meet our full requirements for enrollment by always maintaining your membership in the American Nurses' Association. In order that you may always be filed for active service, it is important that you keep us informed of any change you may make in your name or in your address. The enrollment of nurses failing to report such changes may be can- celled, as provided in our rules.
