My dear Miss Smith:
You have boen nominated for appointment as member of
the Providence, Rhode Island, Chapter Nurse Recruitment Committee,
but since your record reveals your classification as that of the
Third Reserve, an inactive group, it will be necessary for you
to bring your record up to date by providing us with a completed
questionnaire.
In order to be eligible for appointment to membership
on the Committee, a nurse must be in one of the active groups. We
shall appreciate it if you will return the enclosed questionnaire
form as promptly as possible.
