In accord with our present regulations governing the use of the badge and appointment card, Form 741, Mrs. Southworth should be requested to return her badge. This responsibility should be assumed by the nurse enrollment chair- man or her designee. If the badge is retrieved, it should be returned to this office. Jeanne Durr Disaster Nursing Consultent P. S. - Re Mas. Mary Ann Crain, we have also cancelled the enrollment for Mrs. Crain, Badge # 189,638 and will notify the original chapter if we learn of her whereabouts.
