The questionnaire you completed has been received in
our office for filing with your records which we are pleased to
have brought up to date.
In answer to our request for the name and address of
the Chapter in whose jurisdiction you expect to live for the
next twelve months, you have given Poughkeepsie, New York and
have stated that you were returning to the Mainland in November.
We are wondering if you have already arrived in the
States and if so, shall greatly appreciate it if you will send
us your present address and inform us of the type of nursing
activity you are not engaged in.
