Fife, Medical Corps, U.S. Army, Commanding Officer, Base Hospital No. 21, Barnes Hospital, St. Louis, Missouri, for duty: Julia C. Stimson, 788 Mae Auerbach, 6022 Lulu G. Bender, 1361 Byrd C. Boehringer, 8409 Nellie 0. Boothby, 8712 Harriet L. Carfree, 1670 By order of the Surgeon General: Rob to E. Noble Major, Medical Corps Oath of office to be executed upon receipt of orders to mobilize Original in Folder Base Hospital No. 21
