All questions on the application have been answered completely.\n2. CREDENTIALS COMPLETE. Report from school of nursing and necessary references are\nattached\n3. DIVORCE DECREE. Applicant is not divorced; or if divorced, original or notarized\nor photostatic copy of divorce decree is attached that indicates she was the\nplaintiff (brought suit against husband) and that children, if any, were\nplaced in her custody.\n4. CITIZENSHIP. Applicant is a citizen of the U. S. A.\n5. DATE OF BIRTH. Date of birth agrees on all forms on which it appears, and is logical.\n6. AGE. Applicant is less than 45 years of age.\n7. NO CHILDREN UNDER 14 YEARS OF AGE. Applicant has no children, or, if any, they are\nover 14 years of age and applicant is or has been married.\n?\n8. HEIGHT. Applicant is at least 58\" (4'10\") in height.\n7\n9. WEIGHT. Applicant weighs at least 100 lbs.\n10. HEALTH. There is no mention of tuberculosis or venereal disease on the application,\n?\ncredential, or references.\n11.\nREGISTRATION CURRENT. Applicant is registered for the current year and certificate\nnumber is indicated, and applicant is engaged in nursing; or if not engaged\nin nursing, applicant has been registered at some time.\n12. ARREST. Applicant has not been arrested for other than a minor traffic violation.\n13. SUBPOENA. Applicant has never been subpoenaed.\n14. HIGH SCHOOL. Applicant graduated from school of nursing after 1933 and is a high\nschool graduate; or graduated from school of nursing before 1934, and\ncompleted at least 2 years of high school\n15. SCHOOL OF NURSING. Report from school of nursing indicates applicant (a) completed\nher nursing course during 1942 or after, in a school of nursing listed in\nthe\n1944 List of Schools Meeting Red Cross Requirements, or (b) completed her\ncourse prior to 1942 in a school marked with red * in the 1944 List of Schools\nMeeting Red Cross Requirements.\n16. PREVIOUS CONTACT WITH THE ARMED FORCES. Applicant has never served with or been\nrejected by any branch of the armed forces.\n17. EMOTIONAL INSTABILITY. There is no mention of mental illness, nervous breakdown, or\nemotional instability on the application or any of the credentials or\nreferences.\n18. MORALS. Application, credentials, or references do not indicate moral deficiencies\nor questionable traits.\n19. CREDENTIALS.
