In addition to the information that we are asking you to submit on the enclesed form, 615 A, we would like to have the following: relationship of your nearest relative and the name and address of the person or bank to which you wish to have your salary paid mhle you are in England. Also may we know vhom you wish to name as insurance beneficiary. balaries will be paid the first of the month automatically in America. These will be less deductions on a drawing account for personal expenses in England. Your family will be notified of your arrival there and it is the present intention of National Headquarters to issue all pertinent news releases to them. Nail may be sent to the American Red Cross Harvard rield Hospital Unit, Saliabury, England. For your interest 1 em going to ask for $20.00 a month for personal expenses in England. I feel that this sum together with what 1 take with me will be ample. You will be supplied with a white riding shirt which should be kept for special occasions when a group may have to appear in uniform.
