The above-named applicant having applied to be admitted a Citizen of the United States pursuant to the directions of
April 14, 1802, and the Acts subsequently passed on that subject, and to the directions of the Act of Congress entitled passed
the Act of Congress of the United States, entitled "An Act to Establish an Uniform Rule of Naturalization, etc.,"
in Court, producing such evidence and affidavits and making such declarations and renunciations, and taking appearing such oaths personally are
"An Act to Regulate the Immigration of Aliens into the United States," approved March 3, 1903, and
made and recorded, it is
