APPLICATION FOR FINAL PAPERS.
To the Honorable Superior Court,

Now in session at
in and for the County of Windham
John Van den hoort now
resident at
Putnam
in said County, respectfully represents that he was born in
Holland
the
24 th December A. D 1842, and is now about 55
day of
years
of age ; and that he arrived at
new- York
on or
about the 15th day of Octoler A. D. 1872 that it was then, and still is bona fide
his
intention to become a citizen of the Unite 1 States of America, and to renounce all allegiance and fidelity to
every
foreign Prince, Potentate, State and Sovereignty whatever, and particularly to Wilhilmins. Zeen
of the United Kingdom of
Healland
treat Britain and Ireland
whose subject he has heretofore been,
all of which appears on the record of the Honorable
Superior Court, holden at
Putnam in of Windhan on the 16th
and for the County
day of
October
A. D. 1894 a certified copy of which record is herewith exhibited.
AND THE SAID John vanden Goort
further represents,
that he has ever since continued within the jurisdiction of the United States ; that he has never borne any
hereditary title, or been of any of the orders of nobility ; that he is ready to renounce and abjure all allegiance
and fidelity to every foreign Prince, Potentate, State or Sovereignty whatever,
and particularly to Willielmina
Queen of the United Kingdom of Heollend
Britain, Treland
whose subject he has heretofore been ; that he is attached to the principles of the Constitution of the United
States of America, and well disposed to the good order and happiness of the saine.
WHEREUPON, he prays that he may be admitted to become a citizen of the United States of America, ac-
cording to the forms of the Statute in such case made and provided. And as in duty bound will ever pray.
