CONDITIONS
Any Check, Bank Draft, or Money Order given in exchange for this receipt shall not be
considered payment of the premium for which this receipt is issued unless such Check, Draft,
or
Money Order is actually paid to the Company on presentment in due course of business.
Agents cannot make contracts binding the Company, nor alter, change, or waive any of the terms or
conditions of the policy or of any receipt or permit, nor waive forfeiture of the policy; nor have agents
authority to collect or receive premiums, except when they are paid in accordance with the terms and con-
ditions of the policy, and then only upon official receipts for the same issued from the Home Office of the
Company in Springfield, Massachusetts, and signed by the President, or a Vice-President, or the Secretary.

NOTICE
The insured is hereby notified that by virtue of this Policy he is a member of the MASSACHUSETTS
MUTUAL LIFE INSURANCE COMPANY, and is entitled to vote either in person or by proxy at any and all
meetings of said Company. The annual meeting is held at its Home Office on the third Wednesday of
January in each year, at 2 o'clock P. M.
