If This\ncertification will be signed in ink by the vender or in case the\nvender is a company or corporation, it will be signed in the name\nof the company or corporation followed by the personal signature of\nthe person acting for the company or corporation, showing the title\nor capacity in which such person signs. The original invoice shall\nbe attached to the original voucher and the copy of the invoice shall\nbe attached to the carbon copy of the voucher to be retained by the\nauditor of the State Administration.\nII REPORTS\n1. The State Administration shall send weekly to the Administrator\nat Washington by telegraph a State summary of the Civil Works pro-\ngram showing specifically:\n1. Date of payroll covered by report\n2. Number of employees on payrolls\n3. Total number of hours worked\n4. Total amount of payroll\nThis telegraphic report shall be followed by a weekly report on\nthe form prescribed by the Administrator and will serve as a basis\nfor future releases of funds to the State Administration.\nThe State Administration shall submit to the Administrator\nat Washington a monthly report, by counties and by projects, of all \nCivil Works expenditures, and such special reports as the Adminis-\ntrator may from time to time prescribe.\n2. The Administrator will prescribe forms upon which the local ad-\nministration shall report to the State Administration the data\nrequired in the above paragraphs.\nARCHIVES AND\nIII GENERAL\nRECORDS\ns\nSERVICE\"\n1. The procedure herein prescribed applies only to the disburse-\nment of funds allotted from the appropriation, \"National Industrial\nRecovery, Civil Works Administration, 1933-1935\", and not to Recon-\nstruction Finance Corporation or local funds.\n2. The special disbursing officer in each State will, upon the nomi-\nnation of the State Administration designate an assistant disbursing\nofficer for obligations incurred directly by the State Administration.\n3. The State Administrations shall provide such additional personnel\nas the special disbursing officers may require for the prompt per-\nformance of their duties in connection with the-Civil Works Program."
